Custom Fields allows users to create customized fields within the Online Scheduling request form. They will be able to add, remove, and create their own fields they would like to collect whenever a patient/customer requests an appointment.

Custom Fields allows users to create customized fields within the Online Scheduling request form. They will be able to add, remove, and create their own fields they would like to collect whenever a patient/customer requests an appointment.

Weave Portal Weave App (beta)
  1. Select Computer.svg Online Scheduling from the left navigation menu in the Weave Portal
  2. Click Custom Fields
  3. On the left side, check the boxes of the fields you desire to show on the Online Scheduling form, including the birthday, gender, and address
  4. To create your own field click on the Plus.svg sign. If you don't need to create fields, skip to step 10 

    Tip: Create your own fields that best fit the needs of the practice. For example, in Vet offices, instead of having First Name, create a field for “Pet Name”.

  5. Type in the label 
  6. Check the box if it needs to be required
  7. Choose a type from the dropdown 
  8. Check the box if the field should be required for New Patients Only
  9. Select Create
  10. When you're finished customizing fields, select Save
